Job Description POSITION TITLE: Permanent Housing Case Manager
REPORTS TO: Caracole House Manager SUPERVISES: N/A POSITION
SUMMARY: This position provides intervention and case management
for HIV+ individuals residing at Caracole's site-based supportive
housing program (Caracole House). Caracole House serves previously
homeless individuals; target populations include African American
men who have sex with men, people who are returning from
incarceration, transgender people, and people who use drugs. This
position fosters independent living through income/employment, life
skills and community engagement. This person works in a team to
actively pursue change behavior in clients who have experienced
trauma and face multiple barriers to health and housing success.
This will be done via individual services and group supports to the
residents. Motivational Interviewing is the key intervention model.
This Case Manager supports clients in increasing independent living
skills and healthy living with HIV/AIDS and co-existing medical
conditions, especially addiction and mental health issues. They
must engage residents with a range of barriers through both
individual and group services. RESPONSIBILITIES:Work closely
cooperatively with a team consisting of case management, housing,
housing quality, residential assistants, program manager, director
of housing, and Caracole administrators to continue to build a
sustainable community.Provide holistic intervention services
including active engagement, assessment, service planning,
referrals, follow-up, and evaluation. o Provide education and
assessment of resident access to information on harm reduction,
safer sex practices, medication adherence and the importance of
linkage to medical care. o Develop a service plan to address
related employment, income, strengths for independent living, and
community engagement utilizing motivational interviewing. o
Directly connect and engage HIV+ individuals in care and related
activities supporting housing stability. o Refer and facilitate
engagement in needed services: life skills, employment, budgeting,
goal setting, payee services.Plan and coordinate with internal and
external resources and/volunteers to offer services for residents
which center on mental health support, addiction, life skills,
employment, budgeting, goal setting, health living with HIV, and
social opportunities with other residents.Facilitate graduation to
other housing opportunities as the client demonstrates readiness
for more independence or a need for increased support.Provide
aftercare: stabilization, support, and engagement to recent
graduates of Caracole House to assist client with managing needs in
the community.Participate in quality management activities.
Maintain annual paperwork, eligibility records, and daily
interaction notes as required by social work practice, regulatory
or funding agencies, and Caracole.Provide client transportation to
medical and other appointments as needed.Participate in
individual/group supervision meetings.Represent Caracole in the
community. QUALIFICATIONS: High school diploma or equivalent with
at least 3 years' experience working or volunteering in a social
service setting. Demonstrate cultural competency and sensitivity to
individuals experiencing homelessness, with mental health or
substance use issues, living with chronic disease, and/or
experiencing incarceration. Personal experience with these life
issues is valued. Proven ability to work successfully with staff,
clients, their families and support systems, interdisciplinary
teams, and community agencies. Must have personal transportation.
REQUIRED SKILLS:Meet all required deadlines for
documentation.Maintain a collaborative working relationship with
Caracole team members.Ability to maintain confidentiality in
verbal, print, and electronic communications.Must possess excellent
skills in interviewing and engaging clients; experience with
Motivational Interviewing desirable but not required.Must be able
to speak, read and understand spoken English with good verbal and
written communication skills.Basic computer skills, including
familiarity with Microsoft Office. PHYSICAL REQUIREMENTS: This
full-time hourly job requires working fully in office and moving
between and within two three-story buildings that are next door to
each other. The regular schedule will be Monday through Friday from
10am-6pm. The employee is frequently required to sit, stand, walk,
talk, hear; use hands and fingers, handle or operate objects,
operate a telephone, fax machine and computer. Must have driver's
license and insurance and vehicle in order to transport clients and
travel to other sites in the community. The employee must be able
to lift and/or move up to 25 pounds. COVID 19 vaccination (or
waiver) required upon hire. SALARY AND BENEFITS This position is
full time, hourly: $21.63/hr. (Approx. $45,000 annually) . Benefits
