Recovery is seen as a personal journey -that -utilizes -supportive relationships, empowerment, social inclusion, -and coping skills.Position Responsibilities: The Early Connections Case Manager II will provide pregnancy, postpartum, infancy, and early childhood services to patients and their families. These services will include on-going assessment, case management, care coordination, skills training, supervised visitation, and parent education. Services will be offered in the most appropriate setting including school, home, community, and clinic. -Qualifications:

• Bachelor degree in Social Work, Psychology, or other mental health related field -
• One (1) year experience providing direct services to clients in a mental health setting is preferred.
• Relevant professional and life experience may also be considered in lieu of degreeDay-to-Day Expectations:
  • Implements activities for the patients and the families in Early Connections. -
  • Provides therapeutic services for patients and families involved in Early Connections including home based services, supervised visitation, and parenting services. -
  • Provides transportation to treatment activities and appointments, when necessary.
