Description: DEPARTMENT\: Primary Care SUPERVISOR'S TITLE\: Clinical Manager; Medical Director TITLES SUPERVISED\:N/A TOTAL NUMBER OF FTE's DIRECTLY OR INDIRECTLY SUPERVISED\: 0 PURPOSE OF POSITION\: To function at the direction of; and in conjunction with; physicians and nurse practitioner's in providing direct care to infant; child and adolescent patients and their families within the parameters of the Performance Standards for Practical Nurses. REPRESENTATIVE RESPONSIBILITIES--Care DeliveryUtilize the plan of care to provide nursing care to patients and families as assigned by, and in conjunction with the Registered Nurse (RN) and Advanced Practice Nurse (APN).Obtain and communicate patient data for inclusion in the overall and ongoing patient assessment. Assist with development of the plan of care and actively participate in patient care conferences. Provide direct patient care to patients in accordance with current standards of practice and hospital and nursing policies, procedures and standards. Assist in planning for discharge of patient; participate in patient and family teaching.--Quality ImprovementActively participates in improving departmental performance through recommending areas or approaches for improvement activities, supporting new processes, collecting data and providing input to department improvement projects. Possess basic knowledge of process/quality improvement science and tools.--DocumentationDocument results of care given and contribute to the evaluation of the effectiveness of care.--CollaborationWorks with others (patients, families, and healthcare providers) in a way that promotes/encourages each person's contribution. Works to create a positive environment for patients, families and co-workers. Demonstrates flexibility to meet the daily needs of the department.--Customer CareCreates an environment consistent with family-centered/medical home models of care. Actively assess the patient experience and works to identify, mitigate or resolve problems or issues before they occur. When problems do arise, mitigates and resolves to the patient's/family's satisfaction. Investigates and resolves customer requests, petitions or problems. Receives telephone calls from customers inquiring about services or having problems. Talks to the customer to learn questions and sources of problems and works to actively and promptly resolve problem. As needed, contacts outside representatives to request information or assistance in resolving problems.--Family Centered CareProvides care consistent with the family-centered care and medical home models. Insures that care is provided in a manner that is flexible, culturally competent and responsive to the needs of the patient and family. Families are treated as full partners and included in all decision making regarding their child's care. This includes offering information, services and support in a manner that is collaborative and respectful.--CommunicationCommunicate with patients, families and employees in courteous manner. Ensures that patient/family communication is unbiased, cultural competent and explained in a way that the family understands. Responds promptly to requests by patients, families and employees. Offer assistance to co-workers after completion of own duties. Take measures to ensure confidentiality of patients, families and employees. Participates in identification and resolution of work situations in conjunction with staff or designee Presents status or changes in patient condition on an ongoing basis. Documents information appropriately in patient medical record.--Environment of CareAssumes responsibility for orderliness, correct storage of supplies, and communication with environmental services for maintaining a clean and safe environment. Prepares registration, waiting, examining rooms, weigh stations and work rooms daily, restocking as necessary. Assist in the maintenance of clinic equipment, including the washing of instruments, toys and preparation of other supplies. Understands and adheres to all applicable TJC and OSHA standards.--FlowCoordinates and assists with the timely flow of patients and families. Monitors individual patient cycle times to ensure that patients are seen and treated as quickly and appropriate as possible and that rooms are turned over as quickly as possible. Identifies bottle necks and works independently or as a team to resolve or remove.--Fiscal ResponsibilityConsistently looks for ways to improve productivity, reduce costs while producing the same or improved outcomes and experiences for patients and families. Insures that all patient chargeable items are charged. Appropriately documents removal of medications and immunizations from Pyxis. Qualification: EDUCATION/EXPERIENCE
Required\: --Current License to practice in the State of Ohio
Preferred\: --1 year of pediatric nursing experience
