JOB DESCRIPTION
Program Coordinator / Caregiver Supervisor
General Duties: - Management and coordination of services to people
living in their own home in accordance with supported living
philosophy and the regulations of the Medicaid Waiver Programs.
-Direct services are provided in the home of the individual and/or
family and comply with FLSA.
Responsible to: - Program Manager and Person Served
Specific Duties:
Provide direct services in accordance with the My Plan and written
procedures for each person served.
Will be responsible for providing at least one 8-hour shift of
direct service per week
Participate in Quality Assurance Reviews and follow-up according to
rules #5125:2-12-02 Provider Certification and 5123:2-12-01
Supported Living Quality Assurance, as well as Quality Assurance
for Medicaid Waiver programs per the standards and assurances for
those programs.
Implement and maintain documentation necessary for all Medicaid
Waiver programs.
Complete application and re-determination packets for persons
utilizing a Medicaid Waiver program as required.
Supervise direct service providers to include: -interviewing and
hiring; orientation and training; regular written evaluation; and
disciplinary action as per personnel policies.
Schedule live-in/relief/drop-in companions per house as set by the
My Plan or as needed.
Attend all meetings involving the individual (My Plan's, Behavioral
and vocational) as requested.
Monitor services provided to the individual by developing a
relationship of trust with the individual, and together evaluating
the quality of the service.
Conduct visits to each consumer's home as directed by supervisor
(weekly, twice-weekly)
Conduct after hours (6pm-12am) visits to consumer's home on a
monthly basis
Respond to emergency situations on an "on-call" basis per standards
and/or policies of the company.
Participate in the orientation and training of employees throughout
the company as needed and initiate obtaining other training as
needed.
Maintain driving eligibility per policies and provide
transportation as needed.
Actively coordinate efforts for inclusion in the community
including increasing the person's circle of support.
Implement and monitor the individual service plan (My Plan) to
include:
Assessing current skill levels of individuals
Development of training procedures/activities
Following up on all services (assessments, benefits, etc.)
identified in My Plan.
Monitoring the Individual's finances per the My Plan and company
policies and procedure
Communicate with parents, co-workers, staff and outside agencies in
positive and professional manner.
Monitoring all medical needs, information and appointments per the
My Plan and company policies and procedures
Monitoring documentation per the My Plan and company policies and
procedures.
Monitoring health and safety issues of each household.
Other duties as assigned by supervisor.
Essential Knowledge, Abilities, and Skills:
Knowledge of supported living philosophy, and behavior management
techniques.
Ability to work with developmentally disabled and/or physically
disabled individuals.
Ability to supervise employees.
Qualifications:
Supervisory skills and experience with individuals with
developmental disabilities. -Bachelor degree in related field
preferred.
Valid driver's license, good driving record, reliable
transportation and proof of full liability coverage.
