Job DescriptionBrightStar Senior Living of Mason is an Assisted Living / Memory Care community located in Warren County. Our 41 bed community offers all the amenities of home with low ratios and a family atmosphere. Position Responsibilities Summary:The Director of Health and Wellness ensures proper clinical oversight and safety of BrightStar Senior Living residents, educates and provides care and support to residents and their families and provides training/educational opportunities to the community's staff.Key Leadership, Management and Accountabilities (LMA):The Director of Health and Wellness conducts resident assessments, creates individualized service plans and participates in service plan reviews, manages resident electronic health records, acts as the residents' advocate, educates and provides care and support to residents and their families, and provides training/educational opportunities to the community's staff. As with others on the management team, the Director of Health and Wellness is responsible for providing on-call support to our direct care staff.

  • Conducts assessments, reassessments, and creates individual service plans. Participates in service plan reviews with residents, potential residents and their families.
  • Maintains, updates and audits each resident's health record.
  • Assists in the admission and discharge processes.
  • Conducts Resident at Risk Meetings, ensuring follow through on identified plans to minimize negative outcomes
  • Ensures medication management/oversight is done in a comprehensive, competent fashion
    • Educates and provides care and support to residents and their families.
    • Acts as residents' advocate.
    • Provides training and educational opportunities to the community's staff.
    • Educates residents and their families.
    • Promotes safety, education and accountability in the care and support of our residents.
    • Assists community care team in maintaining compliance with OH Residential Care Facility regulations, OSHA and HIPAA regulations.
    • Stays up to date on all life safety/emergency policies and procedures and participates and assists in drills within community.
    • Builds relationships with residents and their families.
    • Assists in the development and implementation of the nursing budget, ensuring clinical excellence, regulatory compliance and proper infection control.Education/Certification Requirements:
      • Possess a current, active and unencumbered license as an LPN with the State of Ohio. Has completed a nursing management course with a clinical component or who has at least one (1) year of nursing supervision in the past five (5) years.
      • Two (2) + years of recent experience working in a healthcare role as a nurse, preferably from within a Long-Term Care, Assisted Living and/or Memory Care facility.Skills/Qualifications Requirements:
        • Strong customer service and communication skills (English).
        • Experience in a leadership or managerial position preferred.
        • PC proficiency (including MS Office- Outlook, Word and Excel).
        • Experience with electronic health records.
        • Valid driver's license and auto insurance.
        • Ability to physically push a resident in their wheelchair distances of 50 feet or more on a regular basis. Travel Requirements:
          • Limited travel as needed for various staff and client care service needs.
          • Up to 10% or as directed, based on the needs of the business and attending job required training and conferences. Work Environment:
            • Office Environment: fast-paced office/community environment; continuous phone use/ringing; talking; various noise levels; interactions with employees and residents; sitting and standing for long periods of time; bending and stooping required; lifting up to 10 lbs. regularly and up to 50+ lbs. may be required; using a computer for extended periods: typing, staring at a bright monitor or TV screen; using fingers, wrists, and arms; stress on back may occur; possible exposure to prescription drugs, bodily fluids, small appliances for cooking, cooling, and heating foods or securely storing medications.
