Pay Range: $24.00/hr – $27.20/hr

What You'll Do:


Assess and support the social and emotional needs of residents and families


Develop individualized care plans with the interdisciplinary team


Provide one-on-one support and counseling


Lead education sessions and connect residents with resources


Play an active role in enhancing quality of life across the community


What You'll Bring:


Bachelor's Degree in Social Work (preferred)


Licensed Social Worker (LSW) in Ohio – required


Experience with mature adults preferred


Empathy, professionalism, and a strong sense of purpose
