Job DescriptionPRN, 1st shift, 16 hours bi-weeklyDemonstrate
respect, dignity, kindness and empathy in each encounter with all
patients, families, visitors and other employees regardless of
cultural background. Responsible for providing consultation and
coordination of the care of patients requiring wound care
assessment, management and treatment (Involves skin, wounds,
ostomies, pressure sores, incontinence and other wound care
arenas). Collaborates with nursing staff, medical staff and other
health care providers in establishing and executing policies,
procedures, and standards of care for wound care management
patients. SECTION II - KNOWLEDGE, SKILLS & EXPERIENCE - Include

begin working in this position and the additional knowledge and
skills that are desirable, but not essential.
MINIMUMDESIRABLEEducation, Credentials, Licenses: Graduate from an
accredited school of nursing. Licensed to practice nursing in the
state of Kentucky. BSN or currently enrolled in BSN program. Meets
contact hour requirements for licensure, including all
state-required courses.MSN; Certification as a wound, ostomy,
continence nurse (WOCN).Specialized Knowledge: Basic knowledge in
current skin, ostomy and wound care practices. Understanding of the
organizational structures of the hospital, its policies and
practices. Able to be self directed and highly motivated.Graduate
of Enterostomal Nursing Education Program. Certification as Wound
Ostomy Continence Nurse. Advanced knowledge and experience in skin,
ostomy and wound therapy and practices. Ability to interface wound
care management into daily practices by staff.Kind and Length of
Experience: Minimum requirements of 3 years current clinical
experience in an acute care facility. Prior experience in skin,
ostomy and would care clinical practice/management.Current
experience as a wound care nurse or enterostomal therapist.SECTION

knowledge and skills for the care and management of patients
ostomies, wounds, incontinence, pressure sores and other skin
related issues.252Serves as an educator, resource person and role
model in the formulation and follow through of the patient's plan
of care as it relates to skin management issue and ostomy
care.153Consults with nursing and medical staff as requested/needed
to evaluate, develop and recommend a management program for the
skin impaired patient to achieve the most positive
outcomes.104Conducts upon request pre-operative assessment(s) for
stoma site selection and counseling for patients scheduled for
ostomy surgery.105Provides input to the Wound Care Manager re:
revisions of current hospital policies, procedures, and standards
based on best practices and current clinical research and
practice.106Provides in collaboration with the Wound Care Manager
in-service education programs related to skin care management and
ostomy care to staff as requested/needed.107Assists physicians and
nursing staff in coordinating care of patients during their acute
hospital phase, and on an outpatient basis.58Consults with Home
Care on patients with skin care/wound/ostomy problems and issues.
Makes home care visits as needed.59Assists the hospital with the
financial aspects of skin management by reviewing, tracking and
follow through on products, specialty beds/equipment and invoice
tracking in consultation and collaboration with the Wound Care

follow the applicable safety steps required to perform their duties
to ensure safety for themselves and their patients, and be in
compliance with Medical Center, State, and Federal safety
standards. Examples of such steps may include: performing proper
safety procedures and precautions; reporting safety concerns and
incidents; using correct lifting techniques and disposing of sharps
and infectious waste properly.For Clinical and related positions -
The associate must be able to demonstrate the knowledge and skills
necessary to provide care appropriate to the age of the patients
served on their assigned unit. The associate must demonstrate
knowledge of the principles of growth and development over the life
span focused on the assessment, treatment and care of the newborn,
infant, pediatric, adolescent, adult, and geriatric patients.
Required: Employee must successfully complete an orientation
process that identifies skills needed to practice in their position
and in their job assignment. Skill requirements must be met and
maintained to ensure employee competency. This is achieved through
an annual review of those particular skills and a work improvement
plan for any non-compliant area.SECTION IV - PROBLEM SOLVINGBriefly

for a case management approach in handling patients with skin
management and ostomy needs who have an altered body image.2.
Communicates patients' needs clearly, concisely and in a

staffs roles with patients/families.3. Demonstrates flexibility in
meeting the unpredictable needs of
patients/families/nurses/physicians during a given day and for an
organization that functions 24 hours/day and seven

WORKING CONDITIONSDescribe unusual working conditions such as

flexibility which includes such activities as walking, bending,
stooping, standing, lifting and pulling in areas with varying
degrees of weight. Should be able to sit for period of timeswhile
engaged in office tasks and meetings. Position requires travel in
own car to North, South, Grant County and home care visits as
needed.Safety precautions provide for the potential exposure to
such elements/agents as blood/body fluids as well as toxic and
