Job Description:
*Summary: *Responsible for the coordinationthe MDS process,
based on the comprehensive assessments of the resident's
needs.*Essential Duties and Responsibilities* include the
following. Other duties may be assigned.Completes initial
assessment of residents and all subsequent assessments.Updates
physicians orders as needed.Responsible for entire MDS
process.Reviews MDS assessments when there is a change in a
resident's condition and/or quarterly.Develops Comprehensive Care
Plan with the Interdisciplinary Team.Makes recommendations for
charting requirements.Informs staff of any changes regarding
resident care and care issues.Monitors " RUG" scores foroptimal
reimbursement.May be required to take on-call needs and assist with
routine RN duties.* Maintain the confidentiality of all resident,
staff and organization information.Qualifications To perform this
job successfully, an individual must be able to perform each

Experience* Must have a minimum of two years Long Term Care
